Amenorrhea: When Your Period Disappears
You haven't had a period in four months. You've taken pregnancy tests. All negative. You've Googled every variation of "why did my period stop" and gotten answers that range from reassuring to terrifying, none of which explain what's happening in your body specifically.
Your period used to come without you thinking about it. Now you think about it constantly. You check for signs. You analyze every twinge, every mood shift, every change in your skin, looking for evidence that your cycle is coming back. The silence from your body is unsettling in a way you didn't expect. Something that used to feel automatic has stopped, and you don't know why.
You may have been told to wait it out. You may have been told to go on birth control to "get your period back." You may have been told your labs look fine and there's nothing to worry about.
Your body stopped cycling for a reason. Finding that reason is what changes things.
What Is Amenorrhea?
Amenorrhea is the medical term for the absence of menstrual periods. There are two types.
Primary amenorrhea occurs when menstruation has not started by age 15, despite normal growth and development. This is less common and is typically related to genetic conditions, hormonal disorders, or structural differences in the reproductive system.
Secondary amenorrhea occurs when someone who previously had regular periods stops menstruating for three months or more. This is the type most women experience, and it is often related to stress, hormonal shifts, nutritional status, or underlying health conditions.
Research published in the Journal of Clinical Endocrinology & Metabolism found that secondary amenorrhea affects approximately 3 to 5 percent of reproductive-age women, though the actual number may be higher because many women delay seeking evaluation. A missing period can feel like a minor inconvenience. It is a clinical signal that deserves attention.
Why Your Period Stops
Your menstrual cycle depends on a coordinated conversation between your brain, your pituitary gland, your ovaries, your thyroid, and your adrenal glands. When any part of that conversation is disrupted, ovulation stalls and menstruation stops. The causes vary, but the body's logic is consistent: when conditions are wrong for reproduction, the system shuts down.
Stress and hypothalamic amenorrhea. This is the most common cause we see in our practice. When your body perceives ongoing stress, the hypothalamus reduces production of GnRH, the hormone that triggers the cascade leading to ovulation. Research published in Psychoneuroendocrinology has shown that elevated cortisol directly suppresses reproductive hormone production. The stress can be psychological, physical, or both. Major life changes, grief, burnout, chronic overwork, and emotional trauma all qualify. Your body can't distinguish between types of threat. It responds to all of them the same way. Undereating and energy deficit. Your body requires adequate energy to maintain reproductive function. When caloric intake is too low relative to energy expenditure, the hypothalamus suppresses ovulation. This is common in women with restrictive eating patterns, but it also affects women who don't think of themselves as dieting. Skipping meals, intermittent fasting taken too far, or simply not eating enough to support your activity level can create a deficit significant enough to shut down your cycle.
Excessive exercise. Intense training combined with inadequate fuel creates an energy deficit that triggers the same hypothalamic suppression. This is common in competitive athletes, dancers, and women who exercise intensely as a form of stress management.
PCOS. Polycystic ovary syndrome disrupts ovulation through hormonal imbalance, elevated androgens, and insulin resistance. Cycles can be very long, very irregular, or absent entirely. Thyroid dysfunction. Both hypothyroidism and hyperthyroidism can cause amenorrhea. Thyroid hormones influence every step of the reproductive cascade. Coming off hormonal birth control. After years of suppressed ovulation, the system that produces follicles, triggers ovulation, and generates progesterone may take time to recalibrate. Premature ovarian insufficiency. POI occurs when ovarian function declines before age 40. Symptoms may include missing periods, hot flashes, night sweats, and difficulty conceiving. Unlike menopause, ovarian function in POI can fluctuate, making diagnosis more complex.
Why a Missing Period Matters
A missing period is more than an inconvenience, even if you're not trying to conceive.
Bone health. Estrogen maintains bone density. Research published in Obstetrics & Gynecology found that women with prolonged amenorrhea have significantly reduced bone mineral density, increasing the risk of osteoporosis and stress fractures. This is especially important for younger women who are still building peak bone mass.
Hormonal health beyond the cycle. The hormonal disruption causing amenorrhea affects more than your uterus. It affects your sleep, your mood, your energy, your cognitive function, your skin, and your cardiovascular health. Addressing the underlying cause improves all of these.
Fertility. If amenorrhea is caused by absent ovulation, conception cannot occur. Many women don't realize the connection until they're ready to conceive and discover their cycle hasn't returned. Identifying and treating the cause now creates the foundation for future fertility, whether that's months or years away.
Why Standard Approaches Often Fall Short
The most common medical response to amenorrhea is hormonal birth control. The pill creates a withdrawal bleed on a predictable schedule. This bleed can feel reassuring. It is not a true period. It is a pharmaceutical bleed triggered by hormone withdrawal. The underlying cause of the amenorrhea continues underneath it.
When you stop the pill, you're back where you started, often with additional delay as the hypothalamic-pituitary-ovarian axis recalibrates.
Provera challenges (a short course of progesterone to trigger a bleed) are sometimes used diagnostically. If you bleed in response, it confirms your uterine lining is responsive to hormones. It doesn't address why ovulation stopped.
For hypothalamic amenorrhea, the standard recommendation is to eat more and exercise less. This is accurate and also incomplete. It names the input (more energy) without addressing why the nervous system is keeping the reproductive system suppressed. Many women increase their food intake and reduce exercise and still don't get their period back, because the stress driving the suppression is deeper than calories and training volume. The body needs to feel safe enough to resume reproductive function, and that safety is a nervous system issue as much as a nutritional one.
What Actually Helps
Acupuncture. Research published in Fertility and Sterility has shown that acupuncture influences the hypothalamic-pituitary-ovarian axis, supporting the hormonal signaling that drives ovulation. Acupuncture also reduces cortisol, calms the nervous system, improves blood flow to the ovaries and uterus, and supports the hormonal environment where ovulation can resume. We recommend weekly treatment and tailor the approach based on the specific pattern driving your amenorrhea.
Chinese herbs. Herbal formulas can support estrogen production, nourish blood, regulate the hormonal cascade, and address the specific pattern underneath the amenorrhea. A formula for hypothalamic amenorrhea looks different from one for PCOS-related amenorrhea. The treatment matches the pattern.
Nutritional rebuilding. If energy deficit is contributing, increasing caloric intake, fat, and protein is essential. Your body needs cholesterol to produce hormones and adequate energy to prioritize reproduction. This is where the relationship with food matters as much as the food itself. How you eat, when you eat, and what your body believes about safety and nourishment all affect whether the system turns back on.
Nervous system regulation. If your body is in survival mode, it will continue to suppress reproduction regardless of how much you eat or how little you exercise. Acupuncture, somatic work, breathwork, and co-regulation all help the nervous system shift from the activated state that's keeping your cycle suppressed.
Stress and lifestyle assessment. Sometimes the amenorrhea is the body's honest response to a life that is asking too much. Sleep deprivation, overwork, emotional strain, and unprocessed grief all contribute. The body will keep telling the truth until the life matches.
What This Looks Like in Practice
A young woman came to us at 18. She had started menstruating at 13 and had regular cycles until she was 15, when she developed a restrictive eating pattern. Her period stopped. Over the following two years, she worked with a treatment team and restored her weight. Her endocrinologist told her that once her body weight stabilized, her period would return. It didn't. He recommended birth control to protect her bones.
She wore oversized clothes to every appointment and sat with her arms wrapped around herself, like she was trying to take up as little space as possible. She didn't make eye contact for the first three sessions.
Her labs read like a woman in perimenopause. Her estrogen was low. Her FSH was elevated. Her body had been in survival mode for so long that it hadn't registered the weight restoration as safety. The hypothalamus was still suppressed.

How long can amenorrhea last? It depends on the cause. Stress-related or exercise-related amenorrhea can resolve within a few months once the underlying factors are addressed. PCOS-related amenorrhea may require ongoing support. Post-birth-control amenorrhea usually resolves within three to six months. If your period has been absent for three months or more, evaluation is worthwhile regardless of the suspected cause.
Can you get pregnant without a period? Ovulation can occasionally occur before your period returns, which means pregnancy is possible even without visible menstruation. However, consistent amenorrhea typically indicates absent ovulation, which means conception would be unlikely until cycles resume. If you're trying to conceive and your period hasn't returned, addressing the underlying cause is essential.
What's the difference between amenorrhea and irregular periods? Amenorrhea is the complete absence of periods for three months or more. Irregular periods mean you're still cycling, but the timing is unpredictable, with cycles that vary significantly in length. Both signal that something in your hormonal system needs attention.
